Job Summary:
The Social Work Case Manager is responsible for assisting the Social Work Department in providing direct and indirect service and assistance to residents and families in matters affecting their psychosocial needs. Assists the Social Work Department in providing comprehensive social work services upon admission, during resident stays, and planning effective discharge plans upon admission. This individual serves as a member of the interdisciplinary team.
